Semen Analysis Interpretation Guidelines
Semen analysis interpretation guidelines are from Dr. Channa to assess natural conception potential, considering female factors.
Lifestyle Factors
- Consider lifestyle factors for all men with abnormal results.
- This includes weight, alcohol intake, smoking, and recreational drug use.
Hormonal Assessment
- Consider hypergonadism in all men with severe fertility impairment.
Key Semen Parameters
- The most important parameters linked to fertility are semen volume, sperm concentration, and sperm motility.
Sperm Concentration
- Men with a sperm concentration below the reference range but greater than five million per millilitre may still conceive naturally, but it may take longer.
- A concentration below five million sperm per millilitre is defined as severe oligospermia and severely reduces the chance of natural conception.
- A concentration below one million sperm per millilitre results in a very low chance of conceiving naturally.
- Azoospermia on two occasions requires specialist urology referral.
Sperm Motility
- A total sperm motility of less than twenty percent indicates severe fertility impairment.
- Consider a genitourinary tract infection in cases of low motility.
Sperm Morphology
- Low sperm morphology is associated with a reduced probability of natural conception. This parameter is less reliable than semen volume, concentration, and motility.
- Men with low morphology but otherwise normal parameters have mild to moderately reduced fertility and may still conceive naturally over a longer period.
Teratozoospermia Index (TZI)
- The TZI quantifies the number of morphological sperm defects.
- A higher TZI is associated with a reduced probability of natural conception.
Round Cell Count
- Round cell count is the total of leukocytes and immature sperm, which cannot be reliably distinguished without further testing.
- Men with a significantly elevated semen analysis report should speak to their doctor.
